Exclude on pages functionality is in my list of updates to the plugin and should be available in the next version. As far as the name change goes; I used this name when I released version 1.0 of the plugin with just 4 options ie : to insert ad above/below/to the left/right of the content. At the time Wp-Insert seemed like an ideal name. Now the plugin has grown considerably but still the name change is not a viable option as many people just search for wp-insert in the plugin installation screen to install the plugin and with a name change wp-insert wont show up in the searches as well as there are a lots of links to the plugin page on wordpress repository which will also become invalid after the name change. @ben : Thanks for sharing your suggestions.
